No — the vast majority of disc herniations (over 80%) improve with conservative management within 6–12 weeks. Physiotherapy, nerve medications, and occasionally epidural injections resolve most cases. Surgery is considered when conservative treatment fails after 6–8 weeks, when there is progressive neurological weakness, or when there are red flag symptoms such as bladder or bowel involvement.
A disc bulge means the disc is pushing outward beyond its normal boundary but the outer ring (annulus) is still intact. A prolapse (also called herniation) means the inner jelly-like material (nucleus pulposus) has pushed through a tear in the outer ring. Prolapse tends to cause more significant nerve compression and symptoms, but both can be managed conservatively in most cases.

Go to emergency immediately if you experience: loss of bladder or bowel control, numbness in the saddle area (inner thighs, groin, genitals), sudden weakness in both legs, or inability to walk. These are signs of Cauda Equina Syndrome — a surgical emergency where hours matter for outcome. Do not wait for an appointment. Call 112 or go to the nearest hospital A&E.
Cervical myelopathy means the spinal cord in the neck is being compressed — usually by degenerative changes or a disc problem. Unlike nerve root compression (which causes arm pain), myelopathy affects the cord itself, and can cause hand clumsiness, balance problems, difficulty walking, and in severe cases, weakness in all four limbs. It is progressive and generally does not improve without surgical decompression. It should be evaluated by a spine specialist promptly.
Yes — in most cases gentle movement and specific exercises help rather than harm. Complete bed rest worsens outcomes for disc problems. Walking is generally safe and beneficial. However, specific exercises depend on the level and type of disc problem: for lumbar disc herniation, flexion exercises are often restricted initially; for some problems, extension exercises are preferred.
